About Us
Prism Community Care has been supporting people with a disability since 2021. We specialise in 24-hour disability programs for individuals with multiple and complex needs across Northern Territory.
Our aim is to be a trusted support provider to people with a disability and children and youth.
Everyone deserves respect and the chance to live their best life. Therefore, our mission is to provide individualised and flexible support for people with disabilities so that they may take part in the opportunities they choose in an inclusive environment. Our creativity and responsiveness ensure that we are the first choice for individuals with disabilities while enabling them to coordinate their NDIS plans and achieve their personal goals. Every person we support is supported to enhance their quality of life and outcomes.
