Supported Independent Living (SIL)

...

Supported Independent Living (SIL) is a service that helps people with disabilities live in their homes with the right level of care and support. SIL aims to empower people with disabilities to achieve their goals and participate in their communities.

At Prism Community Care, we understand that everyone has different needs and preferences when it comes to home and living. That’s why we offer a range of SIL options, from short-term to long-term accommodation, and from shared to individual living. Whether you need 24/7 support or just some help with daily tasks, we can tailor a SIL package that suits you.

Our team consists of qualified and experienced support workers passionate about providing quality care and support. They will work with you to develop a personalised plan that reflects your goals, interests, strengths and challenges.

They will also help you to build your skills and independence, connect with your family and friends, access health and community services, and enjoy a fulfilling and meaningful life.

We offer a variety of support levels, from low to high, depending on your needs and wants. Our support may include:

  • Personal care, such as helping you with showering and dressing
  • Meal preparation, such as cooking and cleaning
  • Help to manage money and household budgeting
  • Assistance with cleaning and laundry
  • Assistance with gardening
  • Skills training, such as teaching you how to manage your money or use public transport
  • Support to access your local community, such as taking you to social events or appointments
  • Assistance with administration of medication, such as reminding you to take your pills or injections
